Client acknowledges and agrees that this 3/D 4/D ultrasound is an elective non medical procedure performed for entertainment purposes only and is not a substitute for standard prenatal care by a licensed healthcare provider.
The ultrasound technician is not a medical physician and will not interpret or provide a medical diagnosis or interpret the images for medical purposed.
The client acknowledges that they are currently under medical care for their pregnancy.
The client understands and agrees that any medical concerns or questions must be directed to their own healthcare provider.
Payment and refund policy:
Due to the elective nature of this service, payment is required in full at start of session. Insurance is not accepted. a $25 non refundable fee will be charged at time of booking.
All payment for services are non refundable. I will make every effort to capture optimal images. If image cannot be obtained due to fetal position or any other reasonable reason, a complimentary rescan will be offered.
Waiver of Liability:
The client releases and discharges any and all foreseen or unforeseen damages, demands, and causes of action related to known or unknown medical condition of client and or fetus.
The client acknowledges that there is no guarantee regarding ultrasound image quality. Image quality is dependent upon fetal position, gestational age, placental location, amnionic fluid volume, and other factors.
the client voluntarily assume all risk of harm or injury to themselves and or their baby resulting from this elective ultrasound procedure.
